Clinical Nurse Consultant (CNC) C 

The CNC C is an expert nurse with advanced theoretical and clinical skills who provides clinical resource, clinical advisory/development and undertakes related projects and development activities to meet specified clinical nursing needs in a specialty area to: 

  • Deliver assessment, consultation and treatment 
  • Manage complex cases / caseloads 
  • Coordinate clinical care and providing clinical supervision or leadership as required 
  • Work collaboratively with multidisciplinary teams 
  • Contribute to strategic planning, team performance, professional development and service improvement activities to continually develop an innovative contemporary mental health service 

CNC C Anticoagulation  

The Anti thrombosis CNC will work with the anti-coagulation stewardship committee and the haematology team in developing a robust Venous thromboembolism (VTE) Prevention Program. The aim is to reduce the incidence of hospital related VTE by ensuring that all patients are assessed for VTE risk and given appropriate prophylaxis. The program provides individual with the tools and resources required to address this significant patient safety issue. This new role will support and provide expert advice as required to implement the elements into workflow.

A clinical leadership role, reporting operationally to the Head of Haematology and professionally to Director of Clinical Operations, the Anticoagulation CNC C is also responsible for developing and implementing projects and strategies to address clinical risks and promote appropriate anticoagulation and prevention of thrombosis. The Anticoagulation CNC C is further tasked with ensuring that the planning, delivery and evaluation of the Monash Health anticoagulation program results in improved health care delivery, consumer outcomes and achievement of performance requirements. Key responsibilities include: 

  • Promoting, implementing and monitoring systems to ensure the safe, appropriate, efficient and effective anticoagulation within evidence-based guidelines 
  • Contributing to the development and implementation of organisation-wide Anticoagulation Haemostasis and Thrombosis management policies and procedures in accordance with national and international standards and guidelines 
  • Developing and coordinating management education and training for the workforce 

Part Time 60.8 hours per fortnight position located at Victorian Heart Hospital!

As an accomplished CNC clinical leader with a passion of VTE stewardship, you are seeking to embark on a new career and excited to be part of an innovative specialist cardiac facility. You take pride in working collaboratively across a supportive and skilled multidisciplinary team to deliver evidence-based, patient and family centred care.  

To ensure your success in this role, you will have following demonstrated skills, experience and knowledge: 

  • Registered with Nursing and Midwifery Board of Australia via AHPRA (Australian Health Practitioner Regulation Agency) 
  • Extensive recent and relevant clinical experience in area of speciality demonstrating appropriate competencies and skills  
  • Demonstrated experience within the acute care setting combined with proven haematology skills / cardiac skill set 
  • Post Graduate Studies (e.g., Graduate Certificate in Transfusion / Cancer / Critical Care) desirable
